Texas House Bill 4224 (89th Regular Session)
Effective September 1, 2025
Under Texas law (House Bill 4224, 89th Regular Session), Donovan Individual and Family Counseling Services, Inc. is required to provide clear and prominent information explaining how Texas consumers may:

Request their health care records from this practice;

Contact the applicable disciplinary or licensing authority governing the provider’s license; and
-
File a consumer complaint with the Texas Office of the Attorney General’s Consumer Protection Division.
This notice is posted on our website and is also available at our facility, as required by law.
1. How to Request Your Health Care Records
Clients have the right to request access to their mental health and treatment records in accordance with applicable state and federal law.
You may request copies of your records using any of the following methods:
A. Online / Patient Portal
Log into our secure patient portal and submit a message titled “Records Request.”
B. Email
Send a written request to:

Please include:

Your full legal name (as it appears in our records),

Date of birth,

Dates of service requested (or state “all records”),

A clear copy or photo of a government-issued photo ID, and
-
Your preferred format (electronic or paper) and delivery method (secure email or mailing address).

Complete and sign our Authorization for Release of Health Information form (available upon request) and submit it in person or mail it to:

Our Response Time
We respond to all records requests in accordance with HIPAA and applicable Texas law. In most cases, we will act on your request within 30 calendar days of receipt. If an extension is necessary, you will be notified in writing with the reason and the expected completion date. Reasonable fees may apply as permitted by law.
Tip for faster processing:
Submitting a completed authorization form, photo ID, and clear delivery preference with your initial request helps avoid delays.

3. How to File a Consumer Complaint With the Texas Attorney General
You may also file a consumer complaint with the Office of the Texas Attorney General (OAG), Consumer Protection Division.
The OAG accepts complaints related to business practices and consumer rights. Please note that consumer complaints are generally not anonymous and require contact information.
